Each Registrant who provides services through our Agency will need the following and can NOT begin working until all of the articles are in good standing:

Personal Identification

  1. Valid Driver’s License

  2. Social Security Card

  3. Vehicle Insurance

  4. Vehicle Registration

  5. Employment Eligibility Document:

  • Voters Registration 

  • Birth Certificate

  • Work Permit

  • Permanent Resident Card

  • Passport

  • Certificate of Naturalization

 

Education

  1. High School Diploma or transcript

  2. HHA or CNA Certificate

 

Medical Information

  1. TB Test or Chest X-Ray (updated every 2 years)

  2. Complete medical physical exam (updated every 2 years)

Background Screening

  1. Local Law Enforcement Background Screening (MUST be on Law Enforcement Letterhead) go to local Police                    Department

  2. FDLE/FBI Level II Background (Live Scan)Can set up through our office

 

License Requirement

  1. CNA, LPN, RN, HHA, etc.

  2. Palm Beach County Home Caregiver ID.

Certifications

  1. Cardiopulmonary Resuscitation (CPR) Certification (updated every 2 years)

  2. First-Aid Training (Can be bundled with CPR)

  3. CNA / HHA Certification and CNA license or “Grandfather Letter”

  4. APD-Zero Tolerance Training  (Must be renewed every three years, Must complete all Modules 1 , 2 , 3 , 4) (Press # for Link)

  5. APD- Direct care core competencies (DCCC)(Must complete all Modules 1 , 2 , 3 , 4 , 5 ) (Press # for Link)

  6. HIPAA Training (Must be renewed annually)(Link)

  7. Self-Administration of Medications Certificate (Must be renewed annually)(Link)

  8. Infection Control Certificate (Must be renewed annually)(Link)

  9. Domestic Violence Certificate (Must be renewed annually)(Link)

  10. Bloodborne Pathogen Certification that is pursuant to Florida State Statute. (Must be from HSI or American Heart)(Link)

Liability Insurance  

  1. Certificate of coverage (NOT mandatory but recommended)
